Facility Letter of Recommendation (notarized)

IMPORTANT: In addition to the above Forms, send a copy of your Photo ID to us by EMAIL and then also with your Application Form. Be sure to add a copy of proof of ‘Completion’ of a NASC approved Surgical Assistant program.

Note: If your graduation date from a NASC approved program reflects a date that is older than 2½ years previous to your applying for the AS-C exam – it will be necessary for you to submit a ‘Surgical Log Form’ below, documenting having first assisted on at least 200 cases a year during the last 2 years along with all other documentation
