Mail in the five Forms below to apply for the RNAS-C Exam (Forms only available in PDF):

RNAS-C Application Form

RNAS-C Code of Conduct Form

RNAS-C Release of Information Form

RNAS-C Facility Letter of Recommendation (notarized)

RNAS-C Surgeon Letter of Recommendation (signed)

IMPORTANT: In addition to the above Forms, send a copy of your Photo ID to us by EMAIL. Be sure to add a copy of proof of ‘Completion’ of a NASC approved RN First Assistant program.

If your Application to sit for the Exam, download the 'Arrange RNAS-C Test Center Form'

Note: If your graduation date from a NASC approved RNFA program reflects a date that is older than 2½ years previous to your applying for the RNAS-C exam – it will be necessary for you to submit a ‘Surgical Log Form’ below, documenting having first assisted on at least 200 cases a year during the last 2 years along with all other documentation
